Referrals

We offer consultations with a Credentialled Diabetes Educator or Diabetes Nurse Practitioner for:

• Newly diagnosed or existing Diabetes

• Type 1 Diabetes, Type 2 Diabetes, MODY, LADA, Type 3c, and ‘other’ Diabetes

• Children and Adults

• Diabetes in Pregnancy

• Gestational Diabetes

To refer a patient to our practice:

• Medical Objects: All of our clinicians also accept referrals directly via Medical Objects
